The Food and Drug Administration (FDA) has warned consumers not to purchase or serve the Once Upon a Farm Tractor Wheels Organic Toddler Soft-Baked Bars, following several reports of young children experiencing complete airway obstruction after consuming them.
The company announced the voluntary nationwide recall of the product, including Florida.
According to the warning published by the FDA, the incidents affected children between one and a half and two and a half years old. When chewed, the product forms a dry, sticky mass that may not detach with standard first aid maneuvers, including back blows and the Heimlich maneuver.
Some cases required the intervention of emergency services and hospital observation.
The recall encompasses all flavors, sizes, lot codes, and expiration dates without exception. The three identified flavors are: apple, sweet potato, and spinach; strawberry, pumpkin, and beet; and banana, pumpkin, and cauliflower.
The product was marketed nationwide in physical stores such as Costco, Target, and Walmart, as well as on Amazon and other online retailers.
Florida is among the states confirmed for distribution by the company, along with dozens of other states and Puerto Rico.
Once Upon a Farm, based in Berkeley, California, stated in its official announcement that the decision was based on three consumer complaints and that it acted "as a precaution, in the best interest of our customers and our commitment to quality."
The company acknowledged that it had not lived up to the trust of families: "We know that parents place their trust in us every time they purchase one of our products. We did not meet that standard, and that is why we are taking these measures."
The company also announced that it will reformulate the product before relaunching it. No other items from the brand are affected by this recall.
What families should do
The FDA recommends disposing of the product immediately without consuming it. Once Upon a Farm offers two alternatives for those who have already purchased it: return the container —unopened— to the store where it was bought for a full refund, or write to the email care@uponafarm.com.
The customer service team responds within up to five business days, and the refund may take up to an additional five business days.
If a child shows signs of choking and first aid maneuvers do not clear the obstruction, the FDA recommends calling 9-1-1 immediately.
This recall is part of a series of safety alerts issued in 2026 regarding children's products sold in major U.S. retail chains.
In July, Target recalled a popular children's product due to injury risks. This month, a liquid supplement for babies sold on Amazon was recalled following concerning findings, reflecting an increasingly active regulatory scrutiny of products intended for young children.
